ABSTRACT

Objective:To explore the construction mode and innovative operation mode conducive to the sustainable development of nursing studio, and evaluate the practical effect of "named nursing studio".Methods:On the basis of carrying out outpatient specialized nursing service in Xuanwu Hospital of Capital Medical University, through further refining diagnosis and treatment service, optimizing treatment process, strengthening personnel training and promoting discipline development, a nursing studio named after nursing experts was formed, quality control standards and long-term follow-up were established, and a complete named nursing studio operation system was established. The number of nursing clinic patients before and after "named nursing studio" (January to December 2018 and January to December 2019) were counted. The data of patients before and after "named nursing studio", the patient′s satisfaction and other indicators after "named nursing studio" were compared.Results:The number of nursing clinic patients was 1 332 before "named nursing studio" and 1 346 after "named nursing studio". After "named nursing studio", re appointment increased from 63.59% (847/1 332) to 70.51% (949/1 346), and the proportion of patients referred by doctors increased from 10.66% (142/1 332) to 12.93% (174/1 346); the total satisfaction of patients reached 96.97% (993/1 024), forming a trinity of nursing, teaching and scientific research development model of nursing.Conclusions:The development of "named nursing studio" promotes the specialty nursing discipline characteristic and the specialty of nursing service brand construction. It is beneficial to solve the problem of difficulty of outpatient and inpatient care, meets the needs of nursing staff career development, reflectes the value of nurse staff, promotes the development of nursing science.

ABSTRACT

Objective To explore the association of frailty and postoperative complications among older patients with surgery. Methods From November 2016 to November 2017, a total of 245 older patients with surgery were included in this study, demographic data, frailty status and potential risk factors were collected by using general information questionnaire, frailty phenotype and risk factors questionnaire. The outcome was the occurrence of postoperative complications in hospital. The mehod of t test, chi-square test or Wilcoxon-Mann-Whitney test were used for univariate analysis; Logistic regression analysis was used for multivariate analysis. Results The prevalence of postoperative complication among surgical older patients was 25.71%, frailty patients were more prone to postoperative complications in hospital (χ2=51.385, P<0.01). After adjusted of age, activities of daily living, anesthesia classification, serum albumin, hemoglobin and other confounding factors, frailty still as a predictor of adverse outcome, OR=10.978, 95%CI=4.651-25.906 (P<0.01). The time of surgery and American Society of Anesthesiolgists anesthesia classification of could also influence the occurrence of outcome. Conclusions Frailty is an effective predictor of postoperative complication among older patients with surgery. Nurses could evaluate the frailty status about older patient and provide inventions to prevent the occurrence of adverse outcome.

ABSTRACT

This paper aimed to summarize critical elements in smooth switch between conventional surgery and interventional surgery through nursing cooperation of complex spinal vascular malformation compound surgery.①Before operation,nurses should be familiar with the whole surgical procedure,and anticipate special items which might be used during surgery according to location and size of malformation;②Nurses should understand all medical test results which were important basis for intraoperative medication;③Nurses should complete assessment of physical function for postoperative comparison;④Layout of all medical instruments in operating room should be reasonable,and repeated moving should be avoided.⑤Patient position should satisfy requirements of the surgery as well as functional position simultaneously;⑥Measures to prevent pressure injuries should be taken according to patient's intraoperative position;⑦Thrombogenesis should be prevented via connection of vessel sheath and catheter to the pressure infusion apparatus;⑧Internal length and external length of the vessel sheath should be determined according to patient's body shape and lesion location.

ABSTRACT

The guidelines for standard nursing in the PET/MR imaging examination included preparation of examination,injecting drug,observing during scanning,post scanning care,management of adverse reaction of contrast agent and radiation protection.The purpose of these guidelines could were to provide the practical and effective management for nursing care in PET/MR imaging,and offer a framework for nurses that could provide useful and helpful in clinical practice and research.
